I understand that the above mentioned photographer holds full copyright of the photographs detailed above, together with the right of reproduction either wholly or in part.
I hereby grant the Photographer the right and permission to take, use, re-use, publish and re-publish, either separately or together, either wholly or in part, photographic portraits or pictures of me or in which I may be included, in whole or in part, or composite or distorted in character or form, without restriction as to changes or alterations, or reproductions thereof in colour or otherwise, made through any medium, and in any and all media now or hereafter known, specifically including but not limited to print media and distribution over the internet for promotion, art, editorial, advertising, trade, or any other purpose whatsoever.
I understand that the photographs may be used for the following purposes:
- Online, including social media to promote the above mentioned photographer, or used by third party businesses and individuals with the permission of the above mentioned photographer. This could include, but is not limited to, a photo that is submitted to photo competitions, or shared on instagram by news or media outlets or businesses.
- Print, including in brochures, pamphlets etc to promote the above mentioned photographer, or used by third party businesses and individuals with the permission of the above mentioned photographer. This could include, but is not limited to, a hospital being given permission to print and display an image in their maternity ward.
I undertake not to prosecute or to institute proceedings, claims or demands against either the Photographer or third parties in respect of any usage of the above mentioned photographs. I hereby release the photographer named above from all claims and liability relating to images, video or photographs taken of me.
The photographer will not sell or license the above mentioned images to businesses for their unrestricted commercial use. Use will be allowed by third parties at the discretion of the photographer for purposes that align with the Photographer's values (such as birth awareness and women empowerment), and will always be expected to be shared in a way that affords the utmost respect to those featured in the images. Examples might be allowing the private midwife or doula to use photos on their website, or a birth related media outlet to use images in relation to an article about birth photography.
I have read this model release form carefully and fully understand its meanings and implications.
